United Crescent Hill Ministries provides Christmas assistance to families who reside in the 40206 zip code through partnerships with churches, businesses, and community members. We are so excited to get this year’s Christmas Connection program started. We are providing families with $100 gift cards to either Walmart or Amazon (your preference) and $50 Kroger gift cards, for a total of $150 per child. This way, the parent or guardian can decide how this money can best be used to provide a cheerful and fulfilling holiday for their children. 

You may only apply for children within your home in 12th grade or younger. If you are receiving holiday assistance from any other agency or organization, you will not be eligible for this program. *There is a maximum of four children per application. 

Completing an application does not guarantee assistance. Funding is limited and applications will be accepted based on funding availability. We will email you to confirm that we have received your application. We will follow up with requests for documentation, including proof of address and social security cards or birth certificates for each of the children listed on the application. *Additional documentation may be required at the discretion of UCHM staff. 

The application will open on November 1st and close on November 30th.
