Thank you for your interest in the UCHM financial assistance program. United Crescent Hill Ministries is a 501(3)c non-profit that provides outstanding youth programs to children in grades K-8 residing in Louisville Metro and surrounding areas. As a community organization, we believe that quality programming should be available and accessible to all children in our community, regardless of their household income.  As such, we offer financial assistance to any child whose attendance would be affected by their family’s ability to pay the rate of $100/week.

To apply for financial assistance, please complete this form upon receiving. All financial assistance is distributed on a case-by-case, first-come, first-served basis. Upon completing you will be contacted by a staff member at UCHM to confirm eligibility.
